I don't know how you'd integrate any of those exam or quiz extensions into a payment system or to add in voucher system or work it so completing the test is free and you only pay for the results. Perhaps use a subscription system although that's more focused at users paying for content (or membership) up front, and when logged in will see "member only" content.
Joomla is written in PHP so if you're an experienced coder I assume it would be fairly straightforward to develop your own extension(s) to achieve your goal.
