You assign modules through the module manager, assigning modules to menu items (or you can embed them in articles (e.g., with Modules Anywhere (Regular Labs) or the inbuilt "load position" or "load module" which have been in Joomla since day 1 basically).

What sort of "error report" do you mean? Is it something like being able to report 404 errors or similar server errors? You can create custom server error pages like 404's. I suppose that can be expanded to other typical ones like 403 or 500.

I guess you could use custom fields in a form, that can be populated with the current page's URL but I'm not sure how you would do that coding-wise.

Ther might be a form builder with pre-built form templates such as for a "helpdesk request".

I personally use https://extensions.joomla.org/extension/convert-forms/ for contact forms, but not in the context you're talking about. It may have the capability to allow you to populate a field with the page URL.
