Are you following the method of configuration of radio buttons as outlined in their documentation?

Additionally, it would appear BreezingForms is not supported by Joomla 5 - no mention on the JED listing nor on the Crosstec website (the compatibility of the extension goes back to Joomla 2.5 - a version of Joomla that was retired a decade ago). The last update for this extension was over a year ago (May 2023).

Could you turn all error reporting on (set to "maximum") and report any errors back to the forum, also please run the FPA (see the red box at top of screen under Forum Rules).
